Transitions from hospital to residential aged care are critical for safety, dignity, and continuity. Canberra Health Services, in collaboration with sector partners, has implemented a standardised, scalable transition model to address inconsistent processes and communication gaps.
This model delivers:
Comprehensive discharge package including nursing and allied health information
Electronic medication chart
Timely, accurate, person-centred handover across sectors

Reduced medication transcription errors
Strengthened relationships between acute and aged care providers
